Nursing Care Plan and Specimens Essay
Quality is a broad term that encompasses various aspects of nursing care (Montolvo, 2007). The National Database of Nursing Quality Indicators [NDNQI] is the only national nursing database that provides quarterly and annual reporting of structure, process, and outcome indicators to evaluate nursing care at the unit level (Montolvo, 2007). Nursingââ¬â¢s foundational principles and guidelines identify that as a profession, nursing has a responsibility to measure, evaluate, and improve practice (Montolvo, 2007). The purpose of this paper is to analyze the ââ¬Ëmislabeled specimenââ¬â¢ indicator for an inpatient rehab unit and devise an action plan based on best practices to decrease the incidence of mislabeled specimens. Analysis of the data According to Dock, (2005) accurate specimen identification is a challenge in all hospitals and medical facilities. Ensuring that specimens are correctly identified at the point of collection is essential for accurate diagnostic information (Dock, 2005). A mislabeled specimen can lead to devastating consequences for a patient (Dock, 2005). Specimen misidentification can be serious, resulting in misdiagnosis and mistreatment (Dock, 2005). For the second quarter of FY09, the rehab unit met their target of zero mislabeled specimens. The third quarter yielded two actual mislabeled specimens with a variance of two. The fourth quarter actual was one with a variance of one. The first quarter FY10 showed an actual of one and variance of one. For the FY09 the rehab had a total of four mislabeled specimens. This indicator was chosen because of the magnitude of this medical error. Nurses, administrators and laboratory personnel must collaborate and create ways to decrease the mislabeling of spec imens. Nursing plan Nursing interventions to decrease the number of mislabeled specimens and improve actual indicator scores are: 1) Ensuring proper identification of patient 2) The use of electronic technology and 3) Bedside labeling. Each of these interventions will positively impact patient outcomes and reduce errors. According to The Joint Commission [TJC], proper patient identification is best practice for decreasing mislabeled specimens (The Joint Commission as cited by Sims, 2010). National Patient Safety Goal [NPSG], 01.01.01 states that healthcare providers should use at least two identifiers to identify patients. For example, the patientââ¬â¢s full name and date of birth is used to properly identify a patient (The Joint Commission, 2014). According to Kim et al., (2013), developing a standardized specimen handling system has the potential to reduce errors. Figure 1. Steps to properly identifying a patient for specimen collection. The strongest intervention to reduce labeling errors is the addition of barcode technology (Brown, Smith & Sherfy, 2011). The use of automated patient identification and specimen collection techniques can be an additional safety net for routines that are vulnerable to error, especially when coupled with strong systems designs (Brown et al., 2011). Brown et al (2011), found that the clinical applications of electronic and information technology support can assist in the identification, control, and reduction of error rates throughout the process. According to the World Health Organization [WHO] healthcare providers should encourage the labeling containers used for blood and other specimens in the presence of the patient (World Health Organization, 2007). This would suggest labeling specimens at the patientââ¬â¢s bedside or before leaving the room. Nurses should never label specimens before collection as this could lead to serious errors. Summary In summary, NDNQI indicators serve as a schoolmaster that holds nursing accountable to practice. An analysis of ââ¬Ëmislabeled specimensââ¬â¢, found that an inpatient unit had a total of four incidents for FY09. While this does not seem like a great deal of errors, any one error could have detrimental consequences. The plan of action based on best practices is to properly identify the patient, using electronic technology, and labeling specimens at the bedside. After graduating from college, starting a business corporation would be a great idea to earn a clean living. Starting a lucrative business is one such opportuni ty that many college graduates can venture into. Many fresh college graduates have brilliant business ideas. However, these ideas only materialize for the few who are willing to endure the strain that come with initiating a new business (Strauss, 2003). Starting a new corporation requires some information to be created about the business venture. The first and most prominent is the name of the corporation. A name can either be inviting or repelling to potential clients (Strauss, 2003). The name of a corporation needs to be unique and not in use by any other registered body. The name of a corporation uniquely identifies and differentiates it from many other businesses that already exist. ââ¬ËNewline Corporationââ¬â¢ has been chosen as the most suitable name for the business that is being set up in Florida. This is because the name is not in use by any other business and it signifies a new development that is coming up in an already existing industry (Barnet & Ronald, 1974). The next step is to create a filing receipt system. This allows constant follow up of any purchases or payments that have been made. It also creates accountability in the use of capital and resources (Kamoroff, 2000). Knowing the amount of money that has been spent at each stage and on what enables an individual to plan for the remaining tasks. It can also be helpful in time management and in evaluating the progress made in starting up the business. A filing receipt system can be used to determine if it is in line with the specification on when the corporation should be up and running (Beerel, 1993). A filing system enables an individual to easily retrieve information whenever it is required. The filing system consists of both manual and automated systems that are labeled under the month and years of their receivership. The most current files should be put at the top. It is crucial to keep a record of the receipts due to tax purposes. Some kinds of receipts that will need to be kept sa fe include unpaid invoices, business receipts, vender statements and client product receipts (Beerel, 1993). Similarly the airline industry has also been actively participating in this race and many airlines now maintain very attractive and eye catching websites (Weeks and Crouch, 1999; Morrison et al., 1999). The recent years have seen many studies and researches in the field of website assessment (Liu, 2000, Olsina, 1999 and Barnes and Vidgeon, 2001). Although there is a great deal of variation in the website design and content amongst different industries nonetheless quite a few assessment tools with precise excellence criterion have been made (Kim, Shaw and Schneider, 2003). Website evaluation methods used by different authors As the internet has developed very rapidly there has been a number of websites available now. This vastness of information and types of websites makes it a necessity there must be some sort of criteria for the evaluation of these websites so that a proper level of quality can be maintained and junk material could be avoided (Seoyoung and Jinwoo, 2004). In order to carry out this evaluation a study was carried out and the characteristics were collected and analyzed. The contents were divided in to a number of categories so that it becomes easy to evaluate each one. The 4 Ps category division by McCarthy (1976) was adapted as the initial division criterion These 4 Ps were ââ¬Å"product, price, promotion and point-of-saleâ⬠. It has been argued by some researchers that the above mentioned 4 Ps fit properly with the internet related stuff especially websites (Chaffey et al., 2003) however some other authors recommend total substitution of these 4 Ps (Constantinides, 2002; Kotler, 1998). A model for the adaptation of internet commerce has been developed by Burgess and Cooper (1999). The model is composed of 3 point s i.e. endorsement that relates knowledge regarding the corporation provision regarding interaction dispensation linked to online communication Ho (1997) uses an approach for website evaluation that is based upon the approach of phases. According to him the structure of the evaluation criterion is based upon 2 dimensional matrixes. Summer Vacations Essay 1. What I did on my summer vacations. Last summer, we canââ¬â¢t have a lot of relaxing vacations, because I have to move here, but we have a one weekend that my family and I went to Cancun. We had great time together with the joy of swimming, lying on the beach and got a lot of souvenirs. That was my vacation on July. The weather was so hot. My father decided to go to Cancun beach. We bought 5 train-tickets and went on the trip excitedly. That night I couldnt sleep well. I just thought about the interests. We arrived at 9 am. Then my parents rent a house for a hotel. We unlocked our handbag and quickly went to the beach. Dad brought us a kite to fly it. We swam a lot, until 11 am. At lunch time, we ate burgers with bacon. The food was so delicious that my brother wanted to eat more and more. We took a nap for about 3 hours, then got up to meet some friends. Unfortunately, when we arrived at my father friends house, he wasnt at home that time. Some neighbors told us that he had came to the beach to relax. So we decided to go there by taxi. The cost wasnt expensive. When we arrived, my fathers friend was eating a lot. Then we stay there to swime until 6 o clock. At night, we went sight-seeing with some tourist. We saw beautiful natural landscapes and ate ice-cream. We felt pleasant. 2. How my family celebrates Holydays First, my mom and two girls in my family (Mom, grandmother, aunt, cousin etc..) get together to cook an ââ¬Å"epic meal each year we used to go over Xmas eve to 1 of sister-in-laws house for whole family gatheringâ⬠¦. There are 3 kids in my family, did the Xmas tree present thing and all that. Would say Thank You prayer when all done. Now 2 are grown up oldest has 2 little ones so history repeats itself with my grown kids and grandchildren. All at my house, nice midday meal and enjoyment of each other. At night we used to put the presents in the tree, but there is no little kids in the family, so we havenââ¬â¢t wait untill next morning to open them, always is a nice time with the family and we vow to make everyday like Xmas.
